Jack Harold Kolb passed away on June 10th. Jack was born on Oct 17, 1955 to Lorraine and the late Stephen Kolb in Chicago. Jack graduated from Kennedy H.S., University of Wisconsin and an M.A. from University of Illinois. Jack's wife Marie predeceased him. He is survived by son, Oliver; mother, Lorraine; and two brothers, Stephen and Keith. Jack was a loyal Chicago Bears Fan and he'll always be there rooting them on.
